Intake Documentation

Patient Intake Form + Referral + Results (optional)

To confirm your booking, we require key preliminary documents in advance of your consultation. This minimizes your wait time and maximizes your time with your Specialist. Please note that your appointment is not confirmed until we have received your referral and completed patient intake form.

1. Patient Intake Form

A secure message was emailed to you shortly after booking via a separate email address ending with @chatswoodent.xestro.com with instructions to complete our new patient form. It is essential to complete this form as soon as possible before your appointment.

If you are unable to complete the electronic form online, please notify us on (02) 9188 9292. We will note that you will complete a paper form on arrival. Please arrive 5–10 minutes prior to your appointment start time to allow adequate time.

2. Referral

We request a copy of your referral so your appointment can be triaged appropriately. Certain medical conditions require earlier attention, and your referral helps us deliver timely care. You may provide your referral via any of the options below:

  • SMS a Photo: Lay the referral flat in good light, focus the image, and ensure all text is visible. If there are multiple pages, capture each page. Attach and send the photos in response to your appointment confirmation SMS.
  • Email (Attach/Reply): Email a photo or scan to referral@chatswoodent.com.au.
  • Email (Forward): If you received an electronic copy from your referrer, forward that email to referral@chatswoodent.com.au.

Direct Doctor Options: Referrers can transmit documents via HealthLink (chatsent), Medical Objects, email (referral@chatswoodent.com.au), or Fax on (02) 9188 1611.

Note: If you are unable to send your referral in advance, please notify us on (02) 9188 9292 and bring a physical paper copy on the day.

3. Results & Diagnostic Records

If you have undergone scans, pathology, or hearing tests relating to your condition, please inform us prior to your consultation. If you do not hold physical copies, provide details of the test type, facility name, and approximate date. We will request digital access on your behalf (especially for CT and MRI series). Forward digital files or access links to referral@chatswoodent.com.au.

Hearing Loss Symptoms: An audiology evaluation (audiogram and tympanogram) prior to your consultation provides valuable insight for your doctor. While your GP may have already arranged this, we also offer comprehensive, on-site audiology services directly at Chatswood ENT. In many cases, we can accommodate testing on short notice or even on the same day as your consultation.

Snoring & Sleep Symptoms: For snoring, mouth breathing, teeth grinding, or apnoea events, our surgeons frequently review data from the free SnoreLab App. We recommend tracking up to three nights of sleep data. Do not send data in advance; simply bring your mobile device to your consultation.

Paediatric Sleep Observations: Capturing video or audio recordings of your child's sleep disruptions on a mobile phone can provide valuable clinical insight. Bring your mobile device to the appointment.

Fees & Medicare Rebates

Consultation fees are payable on the day of service. Most minor in-clinic procedures (such as diagnostic endoscopies or ear micro-suction) are bulk-billed. A valid GP, dentist, or specialist referral is mandatory to claim Medicare rebates or access bulk-billed items.

  • Standard Initial Appointment: $320 – $380 (Medicare rebate approx. $88.00)
  • Subsequent Appointments: $200 (Medicare rebate approx. $44.00)
  • Existing Patients (Same condition, seen within 2 years): $250 (Medicare rebate approx. $88.00)
  • Existing Patients (New condition OR > 2 years since last appointment): $320 (Medicare rebate approx. $88.00)
  • Telehealth Follow-up Appointment: $100 (Paid in advance, Medicare rebate approx. $45.00)
  • Vocal Cord Check: $250 (Pre-operative check; does not apply to throat or voice conditions) (Medicare rebate approx. $88.00)

Cancellation & Waitlist Policy

If you request an urgent appointment or ask to be placed on our cancellation list, having your completed patient intake form and referral on file enables us to offer you an earlier opening. Patients without a completed registration form and valid referral on file will forfeit priority placement on our cancellation list.

Cancellation Policy

SMS reminders are issued 7 days and 2 business days prior to your consultation. Please reply 'YES' to confirm attendance. If you do not have a mobile phone, our team will reach out by phone or email.

Please Note: Appointments remaining unconfirmed by SMS or phone by 12:00 pm (noon) on the business day prior will be cancelled to allow allocation to patients waiting for urgent care.

If a confirmed appointment is missed without notice, a $100 non-refundable deposit will be required to rebook. This deposit is applied toward your fee upon attendance but is forfeited if cancelled with less than 24 hours' notice.
