Intake Documentation

To confirm a booking, we require key preliminary documents in advance of the consultation. This minimises waiting time and maximises time spent with the specialist. Please note that an appointment is not confirmed until we have received a valid referral and completed patient intake form.

If you are unable to complete the form online, please call reception on (02) 9188 9292. Please arrive 5–10 minutes prior to the appointment to complete paper forms.

Medical Referral

We request a copy of the medical referral so the appointment can be triaged appropriately. Referrals may be provided via any of the options below:

  • SMS a Photo: Reply directly to the appointment confirmation SMS with a clear photo of the referral.
  • Email: Send a photo, scan, or electronic copy to referral@chatswoodent.com.au.
Direct Provider Transmission: Referrers are also welcome to transmit documents via HealthLink (chatsent), Medical Objects, email to referral@chatswoodent.com.au, or Fax on (02) 9188 1611.

Results & Diagnostic Records

If the patient has undergone scans, pathology, or hearing tests, please notify reception in advance. Providing the test type, imaging facility name, and approximate date allows our team to request digital access prior to the consultation.

Hearing Loss Symptoms: An audiology evaluation prior to the consultation provides valuable insight. We offer comprehensive, on-site audiology services directly at Chatswood ENT (often available same-day).
Snoring & Sleep Symptoms: Our surgeons review data from the free SnoreLab App (track up to 3 nights) or video/audio recordings of paediatric sleep disruptions. Please bring the mobile device containing these recordings to the appointment.

Fees & Medicare Rebates

Consultation fees are payable on the day of service. Most minor in-clinic procedures (such as diagnostic endoscopies or ear micro-suction) are bulk-billed. A valid referral is mandatory to claim Medicare rebates.

Cancellation Waitlist Priority

Having a completed registration form and referral on file enables us to efficiently offer earlier appointments as they become available. Patients without complete records forfeit priority placement and are ineligible for cancellation waitlist offers.

Cancellation Policy

SMS reminders are sent 7 days and 2 business days prior to the scheduled visit. Please reply 'YES' to confirm.

Deadline: Appointments remaining unconfirmed by 12:00 PM (noon) on the business day prior will be reallocated.
